The truth about this question is: there is no external way to elevate testosterone and even though some experts suggest diet and exercise, these things won’t make a meaningful and lasting difference. Next, we discussed the loss of masculinity in the world today, and how that has created a culture where people are happy to go with the masses and lack the inclination towards leadership, risk taking and decision-making.
If your testosterone levels are low, there is nothing external you can do with lifestyle to significantly elevate it in a meaningful way. Exercise and diet have a greater effect on your mood and disposition than on your actual body. What has fostered the mindset that it’s possible to boost externally is the media’s need to give us a quick solution. If you’re expecting true physiological changes you’ll be disappointed. When it comes creating the most normal hormonal landscape inside the body, what protocols, substances and dosages make the most impact and ensure that the body benefits? This week, we discuss this with TRT expert, author and founder of All Things Men Dr. John Crisler and discuss optimal treatment protocols and estrogen control.

At the start of the episode, Dr. John told us about his background and how he became involved in medicine with the aim of building a practice that would take care of body builders. Next, we discussed how doctors are managing endocrine systems and what it was like in the early days of the practice.
We also talked about estrogen management and the reasons for the high estrogen problem a lot of young men are facing. John says, “all males are being assaulted constantly by estrogen substances whether it’s soy products, pesticides, BPAs etc.” We discussed optimal treatment protocols and how Dr. John starts with the medical history, “80% of your diagnosis is going to be made on your medical history.” We also discussed how to stimulate the system for patients under 40. We also talked about Clomifene, DHEA, HCG and aromatase inhibitors.
We also discussed the choice between transdermals and shots when it comes to TRT. For a young male, gels mimic normal hormonal levels more closely that shots. They are variable through the day and stable across the week, whereas a shot is variable across the week and stable through the day. Towards the end of the show, we also discussed subcutaneous injections.
HRT for males under 40 requires system stimulation. Whatever protocols you employ should take into account the fact that hormones in younger men are more variable. The best protocol is a combination of HCG, a testosterone and some estrogen control along with DHEA. It’s important to remember that we’re all biochemically unique, so there is no one size fits all approach for any of us

At the top of the show, we got some background on Alison and her path to her career in aesthetic medicine and age management. In terms of the breakdown of her patients, it is 80% women and 20% men, but the number of men continues to increase. Men are interested in procedures like dermal fillers that help to improve the jawline by sharpening it or replacing lost volume.
Next, we discussed neuromodulators like Botox. They are used to control the effects of muscular activity in the face, which lead to creasing and wrinkles. Botox works to relax the muscle groups, and is something that can even be employed to prevent the onset of wrinkles. Alison said, “If you try to start after the fact, it takes a lot longer for the crease to go away.” Alison also shared on other neuromodulators, Xiamen and Dysport. What the right one for you? She says patients have found Dysport to work faster and not be as stiff as Botox so it allows you to maintain natural facial expression. “The point isn’t to have no facial movement, it’s to have no wrinkles.”
We went onto to discuss dermal fillers and how they combat the yearly loss of 1% of collagen from 20, pain control and what your practitioner has to do with it. Towards the end of the show we talked about a new molecule that aids in the breakdown and absorption of dietary fat under the chin, HRT for women and the overprescribing issue in the medical industry right now.
Our facial expressions form lines, creases and wrinkles, and this paired with the loss of facial collagen causes bone and fat pad atrophy. Neuromodulators like Botox, Xiamen and Dysport relax very specific muscle groups and can be employed to sculpt the face again. Derma fillers are also effective in controlling the effects of skin aging. These procedures are meant to enhance natural beauty and masculinity, not leave your face looking stiff. Remember, if someone can point out the work you’ve had done, it’s not good work.

When it comes to back health, what are the biggest misconceptions and what are the exercises that makes the biggest difference in reducing pain? On this episode, we talk to author and doctor of spine biometrics Dr. Stuart McGill and discuss the programs he built to address body deficits and help build a pain free foundation.

At the top of the show Stuart McGill told us about his story and how he got started in his practice. With an interest in optimal spine function, the work he has done relies on his ability to probe a patient’s pain provocation mechanism. This goes against the current medical system trend which, “now revolves around the 15 minute appointment. There is no hope, for the physician to do a thorough enough assessment to arrive at a precise diagnosis of the pain.”
Next we went into detail about his work of assessing pain triggers and the book he wrote, The Back Mechanic. By helping his patients understand the nature of their pain he gives them “the liberation of knowing that you have control over your pain trigger.” We also talked about how Stuart believes in coaching people through pain instead of passing intervention. He also provided his best advice for maintaining back health - walking “in small intervals.”
We went onto touch on the modern workforce and why sitting in our chairs all day puts a lot of strain on our backs. He also stated the danger of going from that to going straight to the gym and how that can do even more damage. “You can have a mobile spine but you can bear really high loads or you can be a power lifter but you have to be stiffened in order to to do that.” To aid your back in the office, avoid overloading your spine for long periods by getting out of the chair, pushing for the ceiling, inhaling and exhaling. This will give your ligaments normal spinal stability. Toward the end of the show, we discussed chiropractic manipulation and Eastern medicine.
It’s very common for strength and conditioning coaches not to measure what that person’s particular body needs. Dr. Stuart McGill stresses the importance of that knowledge. It’s also necessary to remember these key points; the more you trigger back pain you get better at sensing it. After a back injury the body grows more nerves into the disc and every time you trigger pain you trigger easier the next time. Additionally, walking is necessary for back health, “That’s how our bodies were designed.”
